Most people on a GLP-1 start a log, keep it for three weeks, then drop it. The reason is
usually that they wrote down too much. Here is the short list that has stayed useful for me across
two titration steps.

1. Date and time of the injection, not just the date. Weekly agonists have a long tail, and if you
slide from Sunday morning to Wednesday evening over a couple of months you will feel it and have no
idea why.

2. The dose in milligrams AND the volume you actually drew. These come apart the moment you use a
different pen or a different reconstitution. Writing "0.25" is useless six weeks later when you
cannot remember whether that was mg or units on the syringe.

3. Injection site, rotated. Abdomen, thigh, upper arm. Absorption is not identical and a site you
overuse gets lumpy.

4. Side effects with a number, not an adjective. "Nausea 3/10, hours 4 to 10 after dose" tells you
something. "Felt rough" does not.

5. Weight once a week, same day, same conditions. Daily weighing adds noise and nothing else.

The reconstitution arithmetic is where most of the errors happen. If you have a lyophilised vial and
you are adding bacteriostatic water yourself, the units on an insulin syringe do not map onto
milligrams until you fix the concentration, and getting that wrong by a factor of ten is a real and
common mistake. I gave up doing it on paper and now run the numbers through the

A missed dose is worth its own line. Note when you noticed, what you did, and when the next one
landed, because the usual guidance depends on how many days have passed and you will not remember

None of this is medical advice, and the titration schedule itself should come from whoever
prescribed it. The log is only there so that when you do talk to them you are describing what
happened rather than guessing.
